Dag 7

What is Anorectal Malformation ?

During early development of a baby in the womb, the urinary tract, the reproductive tract and the digestive tract all exit the pelvis through a common opening. Normally, separation of these structures occurs. In females there is complete separation of the three systems so that the urinary tract opening is farthest forward (the urethra), the reproductive tract exits in the middle (the vagina) and the digestive tract exits farthest back (the anus). In males the urinary tract and the reproductive tract exit through a single opening (the male urethra) in front, while the digestive tract exits through the anus. An anorectal malformation is a condition in which the rectum fails to separate properly during development and does not migrate into its proper position on the perineum. The anus and rectum lie in front of their normal location and are not located properly in the sphincter muscles of the pelvis. Anorectal malformations occur as a spectrum of abnormalities that ranges from very mild conditions in which the anus is situated only a few millimeters in front of its normal location, to severe anomalies with no visible anus. In the more severe abnormalities, there may be an abnormal emptying of rectum into the genitourinary tract, and associated malformations of the spine, genitalia, and kidneys.

Diagnosis of Anorectal Malformation

Anorectal malformations occur in both boys and girls. The diagnosis of an anorectal malformation can usually be made by inspection. When the baby is examined, the anus is absent or abnormally positioned. Ultrasound and x-ray tests may be used to help in the diagnosis. Additional tests will be obtained to look for associated abnormalities. Spine x-rays, kidney and spinal cord ultrasound, and occasionally echocardiography are all routine tests obtained in patients with anorectal malformations.

Treatment of Anorectal Malformation

The nature and severity of the anorectal malformation, as well as the presence of any associated conditions will determine the surgical treatment that is required. Some patients may be treated with a single operation as a newborn, while others will require operations in several stages. For patients with relatively minor abnormalities, a small procedure such as a cut back anoplasty may be all that is required prior to going home. For other patients, initial treatment consists of a colostomy and reconstruction is performed at a later date. A second operation is performed that separates the abnormal communication with the genito-urinary tract, and properly relocates the rectum within the sphincter muscles in the pelvis. Depending on the type of abnormality present, children may have this operation performed utilizing minimally invasive surgical techniques. Alternatively, a PSARP (posterior saggital anorectoplasty) procedure may be performed. Several months after definitive surgery, the colostomy is taken down, re-establishing gastrointestinal tract continuity

Follow Up of Anorectal Malformation

Following reconstruction, patients born with anorectal malformations must be followed frequently by their pediatric surgeons. For the more severe abnormalities, long-term continence issues are the major reasons for follow-up, while in the more mild conditions, patients and their families must work to insure that worsening constipation does not lead to poor continence. Many patients must be managed with bowel program designed to promote regular evacuations of the rectum to avoid ongoing soiling from chronic fecal retention. With attentive management, and rigorous compliance with prescribed regimens, most patients achieve social continence. Many patients will have normal continence. The Pediatric Surgeons of IAPS (Indian Association of Pediatric Surgeons) treat patients with anorectal malformations and provide long-term follow-up and management for patients and their families.

Arranging blood donor of rare blood groups( rh negative blood types

http://www.rarebloodgroups.org/

Arranging blood donor of rare blood groups( rh negative blood types ) is always a tough task as awareness on rare blood groups is still low and usually getting started on the search for rare blood groups draws a blank with most people.

Hundreds of blood units are required for patients suffering from blood diseases like anaemia, accidental blood loss, excessive destruction of red blood cells, thalassemia, haemophilia, leukemia and bone marrow transplant. There is an acute shortage of safe blood and statistics from most blood banks suggest rare blood groups (rh negative blood types) are very scarce.

The annual collection of blood in India is only 5.5- 6 million units against the requirement of about 8.5 million units.Delhi, Capital of India contributes only 24 per cent to the total voluntary blood donation and situation is more worse in other parts of the country

RareBloodGroups.Org is India’s 1st online registry website of rare blood type donors including AB Negative Blood Group, A Negative Blood Group, B Negative Blood Group, O Negative Blood Group and Bombay Blood Type.

Apart from providing blood donors to sick patients, we are also working hard to create awareness about voluntary blood donation in India..

A Lot of things have been said about Indian railways and its revival. i have seen in news about vari

A Lot of things have been said about Indian railways and its revival. i have seen in news about various initiatives taken by railways and how the railways turned in to profit making entity from a loss making entity. Lalu prasad Yadav, our railway minister is being credit with for spearheading the effort of revival of Railways. Not only the railways turnaround story has become a case study in many of the business schools but Lalu was also invited to give lecture on this story at IIMs.

Some of its efforts such as Internet ticketing, cargo services are very much appreciable and improved railways. But I am very skeptical about the few very important aspects, which are still lacking i.e. cleanliness and infrastructure. I happened to be there on two very important stations on Indian Railways Bilaspur and Allahabad last week. Both of these stations are considered main junctions. My stay at both these stations was prolonged due to some expected/unexpected delay of my train. I just tried to gauge both these stations.

Let me first describe Allahabad station. In my life I have not seen such a worst station. The entire station is just like a mess, unclean tracks and it was a nightmare for me and my wife to stay there for 4 hours waiting for my train. when you look at railway tracks at Allahabad station, you will see all the rubbish things including shit. I had taken a picture of Railway track for posting in this site, but decided not to put it because people seeing the photo may not eat for next two days. So worse the railway tracks are maintained. . Even the stalls there are so unhygienic, it is impossible to buy any product there. One cannot imagine using toilets there. The total infrastructure is in mess.

This shows the ground reality of railways. Why no effort is being taken to clean up the mess here. Why officials are not questioned about such pathetic state of this station. One may answer it is too crowded. But I have seen crowded stations like Mumbai CST and Bangalore. At least they are not so bad and some efforts of keeping the station clean are visible. But at Allahabad, the name cleanliness it self is absent both with officials and travelers. Off course this is my first visit to this side of UP, but I have traveled extensively in south, west, central India and Delhi side.Hence probably I am deep shock after seeing the mess.

On the contrary, I am little impressed with the Bilaspur station at least w.r.t cleanliness. Much better than Allahabad station. What further impressed me is that many of the stalls were given to private parties who have maintained it well. Also the display systems, courtesy of inquiry staffs were good. However the upper class waiting room was very badly maintained with broken chairs. After seeing the waiting room, I decided to spend time in Platform rather than waiting hall.

Why railways not talking about the modernization and improvement of amnesties. Why they are just going behind numbers. My sincere request to Railways is to take up the cleanliness as a main drive and drastically change it. This needs to be top driven and enforced. Few change like usage of suction type toilets as in west to be used instead of open toilets, which invariably makes the track dirty. The entire maintaining and cleaning job of platforms, tracks, toilets, rooms and waiting halls to be outsourced and given to private parties with accountability and deliverables. .All the new trains have to be very modern and comply with all sanitation and hygiene requirements. Otherwise what is the use of so much of profit (if I am right last year Railways generated 15000 Crores.)? It is always said that Railways is for a common man. That doesn’t mean that common man has to travel at such pathetic conditions. I totally agree that a common man has the full responsibility of not spoiling the railway stations and trains, but at the same time Railways has to provide such environment for a common man to think twice before he throws anything at railway station,at least in country like India where discipline is the last thing in a persons priority list. It was said that Kolkata Metro would be a mess after few years in a crowded city like Kolkatta, but look.. Even after so many years it is maintained at same cleanliness levels. So it is incorrect to say that people spoil the places. If a good environment and surroundings are provided, people cannot and will not spoil it.

After seeing Allahabad Station, I wonder whether revival of Railways is a reality or just hype. May be it is real when it comes to numbers but when it comes to ground realities, it is just a faux.

Wake up railways … Clean up, otherwise the next disease epidemic will start from railway station.

Critically yours

Kiran

Study Published in the Journal of Periodontology Suggests Impaired Oral Hygiene May Only Be Part of

Study Published in the Journal of Periodontology Suggests Impaired Oral Hygiene May Only Be Part of the Connection

CHICAGO – June 5, 2008 – Over 1.3 million Americans suffer from rheumatoid arthritis (RA), a chronic, inflammatory disease of the joints. RA is a disabling condition, and can lead to long-term joint damage resulting in persistent pain and loss of function in affected areas. A recent study published in the June issue of the Journal of Periodontology, the official publication of the American Academy of Periodontology (AAP), uncovered yet another potential side effect of RA. Researchers in Berlin, Germany discovered that patients with RA have a higher incidence of periodontal disease compared to healthy controls.

For some patients, adverse RA symptoms may affect manual dexterity, which can make one’s daily routine quite difficult. One area that may be affected is oral hygiene which can ultimately lead to periodontal disease. However, these research findings indicate that poor oral hygiene alone did not account for the association between RA and gum disease, suggesting that other factors may play a role as well.

The study examined the oral health of 57 RA patients and 52 healthy controls. To determine oral hygiene status, each participant underwent a comprehensive oral examination including an assessment of plaque accumulation and gingival inflammation, both indicators of oral hygiene. Probing pocket depth and clinical attachment loss, two markers of periodontal disease, were also measured. Researchers used questionnaires to gauge the subjects’ risk factors for periodontal disease.

The study findings indicated that RA patients were nearly eight times more likely to have periodontal disease compared to the control subjects. These findings accounted for demographic and lifestyle characteristics such as age, gender, education and tobacco use. Researchers then examined the extent to which poor oral hygiene was connected to the increased occurrence of gum disease in RA patients. The results showed that while oral hygiene was markedly a factor, it did not fully explain the association between the two diseases, suggesting that there may be other parameters responsible for the increased prevalence of gum disease in RA sufferers.

“With results suggesting that rheumatoid arthritis is associated with periodontal disease, it is easy to assume that an RA sufferer is perhaps unable to properly care for his or her teeth and gums due to the debilitating nature of the disease,” says Dr. Kenneth Kornman, editor of the Journal of the Periodontology. “However, this study implies that there are other potential factors involved. For instance, both RA and gum disease are systemic inflammatory disorders which may explain the connection between the two. Inflammation is already thought to link periodontal disease with other conditions such as cardiovascular disease and diabetes. We look forward to future research that may reveal the biological mechanisms that link these two important diseases.”

In an effort to best maintain oral health, RA patients are encouraged to brush and floss on a regular basis and see a dental professional twice a year. If gum disease develops, consulting a periodontist is an effective way to determine the most appropriate course of treatment.

According to Dr. Susan Karabin, President of the AAP, maintaining the complete health of RA patients should be a collaborative effort. “It is critical that dental professionals and medical professionals work together when treating a patient living with rheumatoid arthritis. This partnership will assure that both the oral and overall health of these patients is paramount.”

Farakka, June 6: As part of a mission to spread the word that the Ganga needs to be pollutant-free, a team of armymen on boats has for more than a month rowed past floating bodies of humans and cattle, piles of plastic washed up on the banks and streaming industrial effluent discolouring the river water.

The group comprising three officers and 14 jawans of the engineering regiment of the Golden Key Division, Indian Army, had set off on two rowboats from Hardwar on April 14. They were flagged in at Farakka this morning after sailing for over 1,300km.

The chequered flag was waved by Brigadier R.N. Masaldar, the chief engineer of the regiment at the 33 Corps in Siliguri. “Our men had embarked on a unique mission to raise the awareness level of the people living along the Ganga. The other objective of the rowing was to keep this regiment, which works during times of natural calamities, on the move,” the Brigadier said.

Masaldar said water samples had been collected from eight places through which the river flows. “We are going to test the level of pollutants in the river and a report will be sent to the defence ministry.”

Major Ajay Garg, the officer leading the expedition, said if this sort of neglect continued, the river would soon die. After sailing for 53 days, the Major has tales of a river in death row to narrate.

“After Allahabad the water level of the river falls drastically; sometimes it even becomes knee-deep. The level of pollution is worst in Kanpur. But in West Bengal, there is negligible pollution,” said Garg.

The team had spotted several Gangetic dolphin during the trip. “These highly endangered species are threatened by pollution. In the towns and cities we stopped, we tried to impress on the people to stop polluting the holy river,” the team leader said.

Garg said the army was going to seriously monitor the river from now on. “We cannot let the lifeline of north India become moribund or die.” [The Telegraph]

Study Published in the Journal of Periodontology Suggests Gum Disease Affects How People Smile

Study Published in the Journal of Periodontology Suggests Gum Disease Affects How People Smile

CHICAGO – March 19, 2008 – A smile is one of the most universally recognizable facial expressions, helping to depict an individual’s happiness, confidence, attractiveness, sociability, and sincerity. According to a recent study published in the Journal of Periodontology (JOP), the official publication of the American Academy of Periodontology (AAP), a smile may also help convey healthy teeth and gums. Researchers found evidence that periodontal disease, or gum disease, may negatively affect an individual’s smiling patterns and deter someone from displaying positive emotions through a smile. The study, conducted at the University of Michigan, evaluated the smiling patterns of 21 periodontal patients while viewing a segment of a comedy program. At predetermined measurement points throughout the segment, the researchers assessed three dimensions of the patients’ smile: the horizontal width of the mouth in millimeters, the open width of the mouth in millimeters, and the number of teeth shown. In addition, the researchers also noted the number of times the patient covered his or her mouth while watching the segment. Individual perceptions of how the patient’s quality of life is affected by oral health were also considered. The data were then evaluated along with a clinical exam of the patient’s periodontal health.“Since periodontal disease is prevalent in such a large number of adults, we sought to investigate if the disease affects a person’s smiling behavior,” said study author Dr. Marita R. Inglehart. “Smiling plays a significant and essential role in overall well-being. Previous findings suggest that smiling can affect social interactions, self-confidence and can influence how people perceive one another.”The study findings indicated that periodontal disease can impact how a person smiles. The more symptoms of gum disease found in a patient’s mouth, such as periodontal pockets between four to six millimeters deep or loose or moving teeth, the more likely the patient was to cover his or her mouth when smiling or to limit how widely the mouth opened during the smile. In addition, the more gum recession seen in the patient, the fewer teeth he or she showed when smiling. The way patients perceived their quality of life as a result of their oral health was also significantly correlated with the number of teeth affected by periodontal disease.

“It is already widely known that periodontal disease is connected to systemic health,” said Dr. Susan Karabin, President of the AAP. “These results help demonstrate that periodontal disease may affect more than just overall health. It can also impact actual quality of life and make caring for one’s teeth and gums all the more important.”

Obsidian has announced that it will soon commence official training courses for the 'Certified Ubuntu Professional' programme. As per the press release on the Ubuntu website, South Africa will provide leading facilities for training of Ubuntu Linux professionals.

Obsidian has announced that it will soon commence official training for the 'Ubuntu Certified Professional' programme. Obsidian has concluded an agreement with Canonical Ltd, the commercial sponsor of Ubuntu Linux that allows for the company to provide training beginning March 2008.

Robin Edser, Technology Manager of Obsidian comments, “It is fantastic that Ubuntu has reached the level where relevant certified training has become available for Linux professionals. Obsidian looks forward to providing this in the South African market and contributing to the growth in use of Ubuntu Linux in professional applications.”

Obsidian will be providing both Ubuntu Professional Courses 1 and 2, which will assist System Administrators to pass the required Linux Professional Institute (LPI) 101 and 102 and Ubuntu 199 exams in order to achieve the Ubuntu Certified Professional certification. These courses are two of a series of classroom and e-learning courses available for Ubuntu Linux professionals.

Maharishi e Mia Farrow, foto AP

I would like to bring this to the attention of everyone as it is very important.Dr. Zakir Naik's website had the following article published defending his views pertaining yazeed paleed:

Clarification of Dr.Zakir Naik's Statement on Yazeed

It is written by Mr. Manzoor Shaikh, administration manager (of the organisation, I suppose).

He goes on to say:

''5. As far as the Ahl-e-sunnat-wal-Jamaat is concerned, inspite of the difference of opinion it is agreed upon that it is permissible to say "May Allah have mercy on him" or "May Allah be pleased with him" for Yazeed. Therefore saying "May Allah be pleased with him" after Yazeed's name is not Haraam, nor a sin and is not wrong.''

He then provides references of the Fatwas from different Dar-ul-ulooms (listed below) and their image files can be downloaded from here.

He says:

"This has been reconfirmed recently in writing from various Darul Ulooms and Islamic Organisations in India.

Fatawas to clarify and support the above stand.

Fatawas from scholars outside India:
1) Shaikh Abdullah Ibn Jibreen (on audio, written copy to be received)
2) Shaikh al Islam Ibn Taymiyah''

It is a blatant lie and has not been confirmed by the Dar-ul-Ifta of Bareilly Shareef.

When you view the first file here: http://img156.imageshack.us/my.php?image=barelieg8.jpg

You will clearly read on the 5th line, it says:

"Yazeed Kay Liye Mazkoorah Jumlay Isti'maal karna na jaa'iz-o-haraam hai"

To use the aforementioned sentences for Yazeed is Haraam and impermissible.

i.e. "Radi Allahu Ta'ala Anhu" and "Rahmatullahi 'Alayhim"The second Fatwa file is an edited version of the first file and only the last paragraph of the first file has been irrelevantly represented as the opinion of the Aa'lahazrat or Dar-ul-Ifta. You can view it here:http://img142.imageshack.us/my.php?image=summaryoffatawas2xr4.gif

Mr. Manzoor Shaikh, either by ignorance or deliberately, have availed the liberty of misrepresenting Aa'lahazrat's opinion and of Dar-ul-Ifta's by saying:

"This has been reconfirmed recently in writing from various Darul Ulooms and Islamic Organisations in India" ..whereas the complete opposite has been expressed in the Fatwa from Dar-ul-Iftah. -------------------------

Canonical has released the stable version of Bazaar - A Distributed Software Version control system

Canonical has released the stable version of Bazaar - A Distributed Software Version control system for distributed teams of innovative and open source, modern community based software development.
Bazaar 1.0 has been written in Python software language - a software language that is much in demand and commands a lot of respect in the Open Software community - which sadly is lacking in India presently, even though Indian software majors have garnered immense amounts of global outsourcing profits -- but for strategic reasons, are not very happy to promote the spread of free and open source software among Indian schools, colleges and universities due to fear of disturbing their monopolies and outsourcing business models.
Among the features of BAZAAR are :
1. Software developers who are familiar with ECLIPSE Software development platform, can begin to be very productive as they can learn and use BAZAAR easily and in short learning curve.
2. It is based on stable Python API and is compatible with Python versions 2.4 and above. It already has a set of about 20 Plugins, available for immediate use by communities of software developers.
3. No need for coordination with central code base as Commits can be made independently.
4. Features and fixes can be developed in independent branches and merged into main development trunk at any time. This feature is ideal for teaching distributed software development to students in remote locations of India.
5. Software development can be done in creative models, whether as creative and intelligent individual developers, who contribute more code than others, or as cooperating software teams.
6. Dual level of support is available for the GNU GPL licensed Bazaar as users can buy commercial support, or if their software developers and teams are really world class, they can use freely available and generous community support.
