Mon compteur de jours d'externat est confondant aujourd'hui, n'est-ce pas ? C'est parce que cet après-midi, à la seconde même où je suis revenu à la maison, ma soeur s'est exclamée : "Ah ! Maman va être contente que tu sois revenu, elle voulait t'contacter !" Et moi, tout curieux, de demander pourquoi. Réponse: "Elle veut que tu dise que t'es malade ou quelque chose de même pour pas aller à l'hôpital lundi, parce qu'on s'en va en fin de semaine pis on va revenir lundi."

Apparemment, à travers le calendrier surchargé de mon père, le moment est venu d'aller à Montmagny voir Grand-Papa au CHSLD. Stage de chirurgie ou pas, puis-je refuser l'une des deux seules occasions par année d'aller voir mon aïeul ? La réponse est non. La famille avant les études, quand même. Alors je ne prétexterai même pas que je suis malade pour m'absenter, mais plutôt que mon grand-père est malade. C'est complètement différent (et véridique...)

Pour ce qui est de Kronos (le dieu grec du temps) et Oisivia (ma déesse imaginaire des congés et du temps libre) dans le titre de ce billet, ils témoignent de la fin très hâtive de cette journée d'hépato-bile: je suis parti de l'hôpital à midi et demie ! En effet, le seul patron d'hépato-biliaire qui n'est pas en vacances présentement n'avait pas de temps opératoire aujourd'hui, ni de clinique externe, ni de garde. Seule tâche restante ? Torcher l'étage. Et pour ceux qui ne s'y connaissent pas en médecine, ceci ne signifie évidemment pas nettoyer quoi que ce soit, on est pas des préposés d'entretien...

"Torcher" l'étage, ça veut dire faire la tournée des patients, écrire une note d'évolution au dossier de chacun, donner congé à ceux qui vont le mieux, ajuster la conduite pour ceux qui vont moins bien, et réviser les résultats de labos et d'examens de tout ce monde. Ça inclut aussi de régler au vol tous les petits problèmes "domestiques" pris en note par les infirmières durant la nuit: Monsieur Untel vomit, il lui faut du Gravol; Madame Unetelle a encore mal, alors augmentons son Dilaudid; Madame Telle-autre est toute gonflée depuis que le résident de garde l'a bolussée 4 fois en pleine nuit, faut diminuer son soluté avant qu'elle se noie, etc.

Torcher l'étage peut prendre un temps plus ou moins long. Tout dépend du nombre de patients, de la complexité de leurs cas, et aussi beaucoup de la personnalité et de l'empressement des patrons et résidents. En médecine interne dans une unité d'enseignement, on peut torcher un étage de 20 patients continuellement de 8 à 6, à vitesse tortue, en tergiversant tout le temps et en écrivant des notes de 2 pages, au point qu'il reste encore des tâches non terminées quand vient le soir. Inversement, dans certains autres domaines (comme l'urologie) on peut parfois régler la tournée endéans 30 minutes. Aujourd'hui, en l'absence de tout patron d'hépato-bile mais en présence de 2 résidents, ma collègue externe, et moi-même, il a fallu presque 4 heures pour torcher entièrement l'étage. Comme on a commencé à travailler vers 8:30, c'est finalement à midi et demie que j'ai pu quitter Maisonneuve-Rosemont, tout content d'avoir plein de temps libre aujourd'hui. Joie !

Yeah ! Mon bras droit a enfin complètement regagné sa sensibilité !

Aujourd'hui, j'ai vu seulement une partie d'une chirurgie, mais elle fut interminable. L'opération a duré de 9:00 à 15:00. Et de 10:30 à 13:30 (donc pendant trois longues heures !) je me suis retrouvé brossé, tenant la caméra de laparoscopie et tentant de suivre les mouvements des nombreux instruments insérés dans l'abdomen du patient pour cette combinaison ambitieuse : cholécystectomie + gastrectomie partielle + Y de Roux !

Après ces trois heures, franchement, j'étais engourdi de l'épaule droite jusqu'au bout des doigts. Mes deux mains, en fait, souffraient de froid et d'engourdissement, car l'infirmière au service interne m'avait par mégarde donné des gants 7 et 1\2 au lieu d'une paire de 8. Trois heures avec des gants trop serrés, c'est long.

Par bonheur, quand l'anastomose gastro-jéjunale a foiré et s'est mise à couler dans le ventre à 13:30, le patron a décidé de convertir la scopie en laparotomie. Durant les quelques minutes de transition avant ce nouveau chapitre imprévu de l'opération, ma collègue externe (qui avait déjà dîné pendant que j'assistais) a pris la relève et j'ai enfin pu enlever ces gants infernaux et remettre mon épaule droite dans sa position naturelle. Quel soulagement !

En sortant de la laparoscopie-devenue-tomie, je suis allé me chercher un hamburger au resto fast-food qui a paradoxalement ouvert ses portes directement dans l'hôpital. J'ai dîné tranquillement au salon des résidents en jasant avec une autre externe, puis vers 14:30, par un petit relent de CDFose qui flottait dans l'air, je me suis senti obligé de retourner voir ce qui se passait en salle d'op...

Mais heureusement, alors que j'allais chercher je ne sais plus quoi dans mon casier, j'ai aperçu Pavlov qui s'en allait au salon; il m'a malicieusement et frauduleusement convaincu de ne plus remettre les pieds au bloc, et de rester au salon à jaser jusqu'à l'heure de mon cours en fin de journée. On a donc déconné comme d'habitude, calés dans de gros sofas, jusqu'à 16:00, pendant que l'engourdissement de mon bras diminuait tranquillement en picotant. De 16:00 à 17:00, j'ai eu un petit cours sur les pathologies de la prostate, puis hop ! Fini. Une journée de moins d'ici la fin de mon externat 1. Only six to go !

Though veteran character actor Arthur O'Connell looked as countrified as apple pie, looking ever more comfy in overalls than he ever did in a suit. Arthur made his stage debut in the mid 1930s and came into contact with Orson Welles' Mercury Theatre. As a result, he earned the bit role of a reporter in the final scenes of Citizen Kane (1941).

Making little leeway in films, Arthur O'Connell returned to the Broadway lights where he played Polonius in "Hamlet" and Banquo in "Macbeth", finally gaining considerable attention as the amiable bachelor storekeeper in "Picnic" in 1953.

Arthur transferred the role successfully to film three years later and began a series of flawed and forlorn characters on TV and the screen from then on. A particular standout was as James Stewart's boozed up attorney and mentor in Anatomy of a Murder (1959) for which he won his second Oscar-nomination for "best supporting actor" (the first was for Picnic (1956) three years earlier). The mustachioed Arthur O'Connell usually played wise, helpful and friendly, and he also inhabited crafty villains from time to time, but there was always an unhappy ambiance and 'loser' quality in his elderly gents, which made you feel sorry for him. Arthur played Monte Markham's "son" (Markham had been frozen in an iceberg, which explains Markham's young appearance) in the 1967 sitcom "The Second Hundred Years" but the series was short-lived. A popular guest star on all the major shows in the 70s, he was forced to curtail his work load as the progression of Alzheimer's began to steadily creep in. At the time of his death in 1981, Arthur O'Connell was appearing solely in toothpaste commercials.

when i was in the car driving back up to new york, we had to go in this cave! so i was worried that the ends were going to fall down and that we would all be stuck in there and die. and that happend! the cave looked like the donkey kong stage in Mario Kart 64 by the way. so anyways, when it collapsed, everybody that was in their car got out and tried to move the rocks that were in the way of the exit, but that didnt work because they were too heavy! so we all kinda gave up and started to put bed sheets on the ground cause we were tired, then i fell asleep! so when i woke up, i noticed 2 huge windows that were on the cave that we could have just went through if we had noticed them before... then i woke up. EXCITING RIGHT??!?!?!!?! no...

The main story is about the return of the superheroes and their play at the Stage.

Some other stories like one telling about how the Keeper (green Puffle @ Night Club) is now at the Ice Rink or In Focus about penguin Races.

Then one of the coolest things in the newspaper is that thing where you have to choose words and there will come a story, this time it's about the penguin band and how they do something with the Iceberg.

Questionne, examine, écris, re-questionne, réexamine, réécris... La journée a été longue et plate. Je suis surpris d'avoir réussi à faire autant de sourires forcés entre 7:00 et 18:00. Ce matin, on a encore fait une tournée des patients à vitesse tortue incluant une petite réunion de révision des cas avec les patrons; ça a tellement pris de temps qu'on a fini de torcher l'étage à 11:00 !

Il n'y avait aucune salle d'op prévue aujourd'hui, alors une fois l'étage torché, le résident a pensé qu'il serait bien bon d'aller finir la seule et unique consult à l'urgence puis d'aller prendre un café et profiter du temps libre au salon des résidents... Hé ben merde, un de nos patrons était de garde, ce qui de facto signifiait que nous étions tous de garde pour les maudites consult de l'urgence. Et l'urgence a TOUJOURS des consult. En finissant la première, on en a reçu une deuxième. En commençant la deuxième, on en a eu une troisième. Au milieu de la troisième, une quatrième... Résultat: à peine une demi-heure pour dîner.

En début d'après-midi, on a eu une petite demi-heure d'une clinique externe aussi monotone que toutes les autres cliniques du genre: quelques nouveaux cas non-urgents et plein de suivis post-chirurgie. À 14:00 on a pu se sauver pour un cours sur les traumatismes abdominaux, mais quand le cours a fini, vers 15:30, surprise ! La clinique avait avancé à un tel rythme d'acanthaster paraplégique que malgré le petit nombre de rendez-vous (trois fois moins qu'en uro), il restait encore des patients à voir ! Alors j'ai encore fait du QEE, sur deux trucs plates: une plaie à vérifier 1 mois post chirurgie pour diverticulite, et trois minuscules cicatrices au cuir chevelu suite à des exérèses de kystes sébacés.

Encore et toujours aussi lente (parce que, il faut bien l'avouer, le patron était très lent et prenait le temps de paaaaaaaaaarler à chaque patient), la clinique externe n'a fini qu'à 17:00. J'avais franchement hâte de sortir de là, mais il fallait hélas réviser toutes les maudites consult de l'urgence ! Ça a pris une heure, et je suis enfin sorti de ce fichu HMR chaud et humide à 18:00, après avoir baîllé et m'être ennuyé toute la journée.

Je pensais initialement me jeter dans mon lit au plus sacrant, mais en arrivant à la maison j'ai trouvé mes parents dans la cour avec des amis. Ils prévoient un bon gros souper barbecue typique d'une belle soirée d'été, avec de l'esturgeon fumé en entrée... Vous me voyez venir: fuck l'horaire de chirurgie là, hein. Je vais profiter de ma soirée, prendre un bon souper, me tenir éveillé avec du café et du thé vert, écouter du Loreena McKennitt pour chasser de ma tête mon stage trop chronophage, et je ne me coucherai que quand la pleine Lune sera bien haute dans le ciel. Ben oui, je vais être amorphe et irritable demain... Mais ce n'est pas grave: j'entends Tempus qui fait "tic, toc": plus que 7 jours d'externat 1. :P

Cette fois ci, Karina était là un peu plus tôt et on a pu commencer notre longue expérience à temps...
On a mis les tubes à essai dans l'incubateur vers 11h, puis on est allés manger Bruno et moi au restaurant à coté du labo.

Alors que je suis retournée au labo pour continuer mon travail, Bruno m'a laissée pour passer ses premiers examens pour l'obtention de son permis. Et oui, ici au Brésil, avant de commencer le code proprement dit, il faut d'abord faire un test psychotechnique (un peu différent de ceux de France, ici on cherche à prouver que la personne n'est pas folle ou potentiellement dangereuse!), puis une petite visite medicale, suivie d'une prise de photo sur place.

Après la fin de mon expérience, aux alentours de 16h, je me suis lancée dans une grande aventure qui était de rentrer à la maison toute seule en essayant de prendre les trois bons bus!
Une heure et demie après, j'ai passé le seuil de la maison complètement crevée.
De 18h à 23h, je suis restée à discuter avec la maman de Bruno, jusqu'à ce que Bruno et son père arrivent pour raconter leur journée.
Pour être entièrement rassurée, le test psychotechnique de Bruno a annoncé qu'il n'était pas fou et qu'il était apte à continuer. Aucun souci non plus sur la visite medicale... et encore moins sur la photo, même si il avait oublié de se raser le matin!
On est donc allés se coucher vers 23h30, sachant que l'on devait se lever à 6h30 aujourd'hui.

Stage 1 - treat the floor with the wood lye. This will soak into the wood and is left to dry overnight.

Stage 2 – Buff the woca wood floor oil (master oil, colour oil) into the floor. The floor surface should then be rubbed down with coarse patina disks to smooth the wood grain. Any excess oil needs to be removed with a cloth so that the surface is damp rather than wet with the oil. If using the white master oil you will notice that some of the white residue is removed when wiping off the excess.

Stage 3 – Wash the floor using the wood cleaner. Re-oil with maintenance oil and rub down with the medium patina disk

Stage 4 – General maintenance of your floor. The white pigments in the soap will keep the floor light. The white soap should be used regardless of which oil was originally applied.

Preparation with Wood Lye: Stage 1

Ensure your wood floor has been correctly sanded ready for the oiling process.

The Wood Lye is a solution which contains a white pigment that ensures the floor will remain light after it is finished and continues to lighten when exposed to sunlight. You can use Wood Lye on any wood type, both softwood and hardwood. There are 2 types of wood lye which will complement your floor – for softwoods such as Pine and Larch, you need to use the softwood lye. For hardwoods such as oak and exotic species the Wood lye should be used.

Empty the Lye into a bucket or container and stir thoroughly to ensure all the pigments have been fully mixed together (these often settle at the bottom of the bottle). When applying the wood lye use a blue nylon pad with the flat mop to get into tight corners. The instructions on the Woca Wood Lye state that the floor needs to be washed with Intensive Wood Cleaner before applying the Wood Lye, however this is not always required and the floor can be prepared by removing any dust with a vacuum.

1. Spread the wood lye in to the floor using blue flat mop. Apply a generous coat of aprox 7sqm per litre to the floor. Ensure the wood is evenly saturated by brushing along the grain of the wood. Leave to dry for 8 hours.
2. A further wash with intensive wood cleaner is recommended by Woca to remove any excess lye but this step is not always required as this process is also done when the floor is treated with the master oil.

The oiling process: Stage 2

Ensure all the Wood Lye has completely dried out before applying the oil.

It is advisable to wear old clothes during oiling as this process involves using plenty of oil cloths and the oil can damage your clothing. The process takes aprox 1 hour per 20sqm. Take care when using the oil as it can cause skin irritation – the use of oil resistant rubber gloves is recommended. When the cloths have been used they need to be placed in a bucket of water as the fumes can cause self-combustion.

Empty the contents of the oil into a container and stir well to ensure the white pigments are not settled at the bottom of the container.

Apply the oil using the flat mop, brush, roller or cloth. Take care around the edges of the room and apply with a small brush and cloth to ensure no oil is splashed onto skirting or adjoining floors.

Leave the oil for 20 minutes to soak into the wood. If the oil absorbs through before this time apply more oil. Buff the floor using a flat mop to ensure the oil is fully absorbed. Working along the grain of the wood, buff the surface using the green patina disk to smooth the surface. Working from the edge of the room furthest from the exit, wipe off all excess oil using clean cloths and ensure there are no patches of oil left on the floor as this will start to shine when dry. A polishing machine can be used during this process for larger areas. These can be hired from most hire shops. Leave to dry for at least 12 hours and do not walk on the floor in this time. Dispose of all the cloths by placing in a container filled with water as they can self combust / ignite.

Maintenance process: Stage 3

A final maintenance coat should be applied to ensure that the floor is completely protected – if building work is being done protect the floor with Everbuild Roll and Stroll until all work is complete – then apply the maintenance coat.

1st Maintenance Coat
The 1st maintenance coat is applied to ensure that the floor stays durable and wear resisistant. This maintenance coat should be done after all building / decorating work has been completed as it can remove any dirt / wear from work done.

Step 1 – Wet Clean and scrub
This step involves cleaning the floor with Intensive Wood Cleaner. This should be made up of 1 part cleaner and 40 parts water. Scrub the floor using the flat mop to get any dirt off the floor.

Using a dry mop head or dry cloth, wipe off all the excess water from the floor. When the cloth / mop head get saturated rinse and wring out before continuing. Check the floor for marks whilst it is still wet – any marks on the floor can be removed with the edge of a knife.

Leave the floor to dry for 1-2 hours. Some floors including oak floors may look dull now – however the sheen and colour will return when re-oiled.

Step 2 – Oil

Apply a thin coat of maintenance to the floor using a cloth and rub in well. The maintenance oil should be used sparingly. Whilst the floor is still damp, the floor should be buffed by using 2 green patina disks with the flat mop. This will smooth the surface of the wood to give a beautiful protected finish.

Wipe any excess oil from the floor after buffing. Leave the floor to dry for at least 4 hours before walking on it. If you are using a buffing machine and you want a higher sheen on the floor you can apply a second coat of maintenance oil and leave to dry for 1 – 2 hours. When the floor is tacky the excess can be buffed off.

Periodical cleaning: Stage 4

The White soap needs to be used when cleaning and maintaining your floor to keep the floor light. When the floor requires a maintenance clean complete step 3 to bring the floor back to its fully protected condition.

In an attempt to make viewing New York City Broadway shows more affordable for everybody, same-day tickets are sold for some Broadway and Off Broadway shows, and numerous Broadway theaters also suggest special student rates or standing-room tickets to help ensure that more folks have the opportunity to see Broadway shows.

New York City Broadway shows got their begin in 1882, and since then have attracted worldwide recognition and stage names such as Irving Berlin, Julie Andrews, and Jackie Gleason. Claudia, stage estivo in un’agenzia interinale: «Inizio in luglio, ad agosto mi mollano da sola in una filiale del centro alle prese con cessazioni, assunzioni, buste paga, senza nessuna esperienza. E per 200 euro al mese». Massimo, laureato in Scienze della comunicazione, stagista in un’emittente televisiva nazionale: «Dopo il primo contratto di tre mesi mi riconfermano due volte, in totale nove mesi di stage, gratis, soltanto un ticket restaurant al giorno, nessun rimborso spese».

Storie di stagisti delusi, ne circolano un’infinità, il copione è sempre quello: poca o zero formazione, pochi o zero soldi, tempo sprecato. I buoni stage, quelli davvero “formativi” e magari anche ben retribuiti (non è obbligatorio pagare gli stagisti) non sono la regola. Chi li trova, trova un tesoro. La buona notizia è che c’è una mappa per quel tesoro: una guida per scoprire le aziende virtuose è online, nel blog di una giornalista freelance (www.repubblicadeglistagisti.blogspot.com) che ha raccolto le testimonianze dei “reduci” e ha confezionato una “Lista dei buoni” che ha ormai raggiunto quota cento.
Accanto al nome dell’azienda viene indicato il compenso e gli eventuali bonus: contributo alloggio, mensa gratuita o buoni pasto, libero accesso al parcheggio. Ci sono aziende come Procter & Gamble (ottava posizione) e Barilla (44esima) che offrono ai tirocinanti in trasferta la sistemazione in residence, oltre a un rimborso spese fra i 600 e gli 800 euro. La Nestlé (23esimi) a Milano “passa” anche la palestra, e che palestra: gli stagisti hanno libero accesso alla prestigiosa Canottieri Olona.

La prima in classifica, Magneti Marelli paga i laureati mille euro netti al mese che diventano 1200 in caso di proroga e per chi ha un master e offre un rimborso spese di 500 euro netti anche agli universitari, per tutti la mensa è gratuita.
Molte delle premiate ditte promosse dagli stagisti provvedono a sfamare i loro “praticanti” ma non è sempre così. Elena Voltolina, veneziana, 29 anni, è la giornalista-blogger che nel settembre 2007 ha creato il sito con la Lista dei buoni (oggi ha seimila visite al mese) ed è stata cinque volte stagista. «Ho lavorato in un’agenzia di comunicazione per 250 euro lordi al mese e ne spendevo 500 per pagarmi la stanza a Milano. È stato così per me ed è ancora così per la stragrande maggioranza dei giovani. Le aziende che compaiono nella mia lista sono un’eccellenza. Sarebbe ora di mettere mano alla legge, ormai superata. Che cosa cambierei? Fisserei una retribuzione minima di 600 euro netti e una durata massima per i laureati di sei mesi».

La legge che regola questa materia è la 196 del ‘97 (art. 18) con decreto 142 del ‘98: prevede che “il soggetto ospitante non è tenuto a pagare alcuna retribuzione né contribuzione al tirocinante” e fissa il limite di 12 mesi per gli universitari. Ma la normativa risale a dieci anni fa, nel frattempo lo stage è diventato una tappa quasi obbligata per entrare nel mondo del lavoro. E un prezioso strumento per le imprese, che investono tempo e risorse per formare un gio- indivane ma hanno così l’opportunità di conoscere i candidati per poi andare a colpo sicuro al momento dell’assunzione. Gli stage vengono svolti prima della laurea (dalla riforma universitaria del 2001 in poi) e anche dopo. Un’indagine del consorzio inter-universitario Almalaurea rivela che l’11% dei laureati nel 2006 ha svolto un tirocinio al termine degli studi (il tasso di occupazione per loro è del 66%, 13 punti in più rispetto a chi non lo ha fatto). E sono più che triplicati, dopo la riforma, gli stage svolti durante il ciclo di studi: 18% degli studenti nel 2001, 61 nel 2007. «Meglio che il tirocinio avvenga prima della laurea, in questo caso alle aziende è richiesto un impegno maggiore perché devono affiancare un tutor allo studente.

E devono concordare con l’università un progetto di formazione. Gli stage post laurea, senza il tramite della facoltà, possono essere più rischiosi, il nostro consiglio è di concordare prima un progetto di lavoro», dice il direttore di Almalaurea, Andrea Cammelli. «Lo stage è diventato un “pre-periodo di prova”, l’importante è che sia formativo e che non diventi un costo per le famiglie», insiste Eleonora Voltolina: «attenzione alle trappole, soprattutto d’estate ci sono imprese che prendono gli stagisti perché sono a costo zero, basta pagare l’Inail, e fino a 18 mesi dalla laurea ci pensa l’università. Mi ha scritto una studentessa di Scienze della moda: è stata presa coma stagista in una casa di moda, peccato che l’abbiano messa a fare la commessa in una boutique di via Condotti».

COMMESSA A COSTO ZERO In casi come questi dovrebbe essere l’università a vigilare. Molte facoltà ormai hanno uno “sportello stage”. Gli equivoci si creano quando l’accordo fra stagista e azienda avviene senza intermediari. Online i nomi delle aziende da cui tenersi alla larga si trovano, sono in ordine sparso perché nessuno si è preso la briga di compilare un “elenco dei cattivi” e si tratta, come sempre, di notizie da verificare. Godono di pessima reputazione agenzie di pubblicità e di comunicazione, assessorati e uffici stampa, dove sembra che agli stagisti venga inflitta soprattutto la pena delle fotocopie. Ecco perché i ragazzi scaricano dal web l’elenco delle aziende “referenziate”. Claudia consiglia di puntare su quelle “altamente specializzate, giovani e in crescita”: «Lavoro nel settore dell’alta tecnologia, alla Feedback Italia (14° posto nella lista, ndr). Mi hanno assunto dopo uno stage di sei mesi, pagato 900 euro al mese. Qui otto stagisti su dieci vengono assunti». Nella colonna a fianco i primi 25 nomi da salvare.
